Vision & Hearing
Drivers must have at least 20/40 acuity in each eye with or without correction, and be able to perceive a forced whisper at a distance of 5 feet.
Blood Pressure
Your blood pressure must be under 140/90. If you take blood pressure medication, remember to take it on the day of your exam and bring a list of your prescriptions.
Urinalysis
A standard urinalysis is performed to check for underlying medical conditions like diabetes or kidney disease. (Note: This is not a drug test unless specifically requested by your employer).

What conditions could disqualify me from getting a DOT card?
Certain cardiovascular diseases, uncontrolled diabetes, severe vision or hearing loss, epilepsy, and the use of certain medications (like narcotics, methadone, or amphetamines) can potentially disqualify a driver. However, many conditions are acceptable if they are properly managed, stable, and documented by a letter from your primary care physician.
What if I have Sleep Apnea?
If you have been diagnosed with Obstructive Sleep Apnea (OSA), you can still pass your DOT physical. However, you are required to bring a recent CPAP compliance report from your machine. The FMCSA generally requires proof that you use your CPAP machine for at least 4 hours a night on 70% of nights.
Can I pass if I have high blood pressure?
Yes, but your blood pressure must be under 140/90 at the time of your exam. If it is higher, you may be issued a shorter-term certificate (such as a 3-month card) to give you time to lower it with medication or lifestyle changes before being re-evaluated.
What is a Medical Variance or Exemption?
If you do not meet certain physical standards (such as vision, hearing, or insulin-treated diabetes), you might still be able to drive commercially by applying for a Federal or State Medical Exemption. If you operate under an exemption, you must bring your exemption paperwork with you to your DOT physical.

What should I bring to my DOT physical exam?
Please bring a valid driver's license, your current Medical Examiner's Certificate (if renewing), prescription eyeglasses or contacts (if worn for driving), hearing aids, and a list of all current prescription medications. If you have pre-existing medical conditions (like diabetes or sleep apnea), bring relevant medical compliance records or doctor release forms.
How long is a DOT Medical Examiner's Certificate valid?
A standard Medical Examiner’s Certificate (Form MCSA-5876) is valid for up to 2 years for healthy drivers. However, if you have managing health conditions such as high blood pressure, diabetes, or heart disease, the examiner may issue a 1-year or shorter certificate to ensure regular health monitoring.
Do I need to submit my medical card to the California DMV?
Yes. Commercial drivers holding a California CDL are required to submit their valid Medical Examiner’s Certificate (MCSA-5876) to the CA DMV to maintain active commercial status. You can submit your form online, by mail, or at a local DMV field office.
Does a DOT physical include a drug test?
A standard DOT physical requires a urinalysis, but this tests for kidney function, blood sugar levels, and protein—it is not a drug screen. However, many employers schedule a DOT pre-employment drug screen at the same time as the physical exam.
